SUMMARY
Family Service League is seeking a full-time Program Director for the Critical Time Intervention (CTI) Program in Mastic, NY. The Program Director will provide clinical and administrative oversight of a Critical Time Intervention Team serving Suffolk County supporting individuals (16 y/o ) who have serious mental health diagnoses and complex needs which may adversely impact their ability to successfully engage with clinical or community-based services (especially those referred from psychiatric ED/CPEP or inpatient settings) without ongoing, targeted assistance.

RESPONSIBILITIES
- The Program Director will assist with program start up: hiring, training, developing and fostering relationships with community providers and resources.
- Provide clinical and administrative oversight to the interdisciplinary team that includes Master’s level clinicians, certified peers, case managers and a registered nurse.
- Review documentation to ensure policies, procedures and workflows are consistently followed by all staff and in compliance with agency and NYS Office of Mental Health regulations.
- Ensure completion of all clinical documentation in accordance with regulations in an electronic medical record, including progress notes, intake assessments, care coordination, discharge planning and treatment plans.
- Coordinate and connect to participants’ families, natural supports and other community resources.
- The Program Director will lead team meetings, case conferences and group supervisions to ensure the program is operating to fidelity.
- Collect and track program data about productivity and performance indicators. Use data to monitor programming, outcomes and to inform decisions.
- Maintain proficiency in agency Safety and De-escalation training.
- The Program Director will provide ongoing training for new and existing staff.
- Work with Information Technology staff to work with the electronic health record and use of other technology tools and computer applications.
- Ensure the health and safety of staff and clients during emergencies.
- Maintain relationships with hospital systems, community providers and other relevant stakeholders to coordinate client care.
- Support program operations by providing clinical back up when necessary.
- Ensure program has on-call coverage 24/7.
- Attend periodic administrative meetings virtually and off site.
- Provide informative presentations to various community organizations on program services.
- All other duties as assigned.
QUALIFICATIONS
Master’s degree required.
LMSW, LMHC, LMFT or related licensed clinical degree required; LCSW preferred.
At least 5 years clinical experience in a behavioral health setting required.
At least 3 years of supervisory experience required.
Excellent interpersonal skills and strong verbal and written communication skills required.
Ability to problem solve, make clear decisions and work as part of a team.
Proficient computer skills, including Microsoft Office and Electronic Health Records required.
Bilingual Spanish strongly preferred.
Valid and clean NYS Driver’s License required.
Some evening and weekend hours may be required.
PHYSICAL REQUIREMENTS
Ability to sit and stand for long periods of time.
