Family Service League Inc. is seeking a skilled and empathetic Peer Specialist to join our team, working closely with our multidisciplinary team to deliver comprehensive support services to individuals, children, and families. As a Peer Specialist, you will utilize your personal experience living with a disability or social/emotional challenge to connect with clients, build trust, and empower them to achieve their goals.
About the Position
The successful candidate will provide outreach support and coordination of care for clients, ensuring seamless service delivery and working closely with our team to meet the unique needs of each individual. Your strong interpersonal and verbal and written communication skills will enable you to effectively collaborate with our team, clients, and community resources.
Key Responsibilities
* Provide coaching and recovery support to clients, focusing on self-advocacy, linkage to aftercare, wellness self-management, medication adherence, smoking cessation, entitlements, and daily living activities.
* Collaborate with our team to develop and implement individualized care plans, ensuring seamless service delivery and meeting the unique needs of each client.
* Provide telephonic and face-to-face outreach, engagement, and service planning during client stays and immediately following discharge.
Qualifications
* High School Diploma or equivalent required.
* Peer Specialist Certification or eligible with plan to obtain certification within 1 year of employment required.
