Role Summary
We're seeking a highly motivated Recovery Support Worker to join our team at the Certified Community Behavioral Health Clinic (CCBHC) in Riverhead. In this role, you'll work closely with clients to develop personalized plans, provide education and support, and collaborate with interdisciplinary teams to ensure seamless care transitions.
Key Responsibilities
- Provide education and support to clients on topics such as medication adherence, wellness self-management, and daily living activities.
- Develop and implement individualized service plans to address clients' needs and goals.
- Conduct thorough assessments to identify client strengths, challenges, and areas for improvement.
- Collaborate with interdisciplinary teams to coordinate care and ensure seamless transitions.
Requirements and Qualifications
- Bachelor's degree in a behavioral health field required; Master's degree preferred.
- At least one year of related experience as a Case Manager in a behavioral health environment required.
- Familiarity with NYS Entitlements, Housing, Health, and/or Mental Health/Substance Abuse Service Delivery System required.
