Responsibilities
The Case Manager will be responsible for the following tasks:
- Conducting linkage, coordination, advocacy, and mentoring services
- Providing anger management and parenting skills training to youth and their families
- Focusing services on goal achievement surrounding factors contributing to youth's involvement with mental health services
- Carrying a caseload of 14 youth visited weekly
- Referring clients for skill building services and working collaboratively with skill builders to achieve client goals
- Conducting home visits throughout Suffolk County
- All other duties as assigned
Working Conditions:
This position requires sitting for long periods of time at a computer and traveling to conduct home visits.
