We are seeking a highly motivated Social Work Case Manager to join our team at Family Service League Inc. The successful candidate will provide comprehensive services to individuals, children, and families to improve the quality of their lives at home, in the workplace, and in the community.
The ideal candidate will have excellent communication skills, be able to work independently and as part of an interdisciplinary team, and have knowledge of community resources. They will also be able to document all client interactions, track program milestones and outcomes, and participate in agency-wide systems of care and collaborative projects.
Key Responsibilities:
- Provide direct service to target populations, including linkage, coordination, advocacy, mentoring, and skill-building
- Develop and implement individualized plans to achieve program goals
- Collaborate with colleagues to provide comprehensive services
- Document all client interactions and track program milestones and outcomes
- Participate in agency-wide systems of care and collaborative projects
Requirements:
Bachelor's degree in social work or related field preferred. At least 4 years of experience working with families in a related capacity, or a Bachelor's Degree with 2 years of experience. Knowledge of community resources is essential.
