Social Worker Case Manager for Child and Family

About the Opportunity

We are seeking an experienced Case Manager to join our team in the Children's Care Coordination Program. The ideal candidate will have a strong background in social work and excellent communication skills.

Key Responsibilities

• Develop and implement individualized plans for clients with mental illness and/or chronic medical conditions.
• Provide crisis/safety planning in collaboration with mental health providers.
• Advocate for clients and coordinate linkage of services with providers and benefit systems.

Qualifications

• Bachelor's degree in social work and 2 years of relevant experience or Master's degree in social work and 1 year of relevant experience.
• Excellent interpersonal, verbal, and written communication skills.
• Ability to work independently and as part of a team.
• Valid and clean NYS Driver's License.
