About This Role:
This is an exciting opportunity to join our team as a Peer Specialist. As a key member of our multidisciplinary team, you will play a vital role in providing outreach support and coordination of care for clients in our Certified Community Behavioral Health Clinic (CCBHC) in Riverhead.
Key Responsibilities:
- Develop and implement individualized plans to support clients in achieving their goals.
- Provide coaching and recovery support to clients, focusing on self-advocacy, linkage to aftercare, wellness self-management, and more.
- Work collaboratively with healthcare professionals to coordinate care planning and provide expert input on client needs.
- Maintain accurate records and statistics, ensuring timely and organized reporting.
Requirements:
- A High School Diploma or equivalent is required.
- Prior experience as a peer specialist in a related setting is preferred.
- The ability to draw upon personal experience living with a disability and/or social/emotional challenge is essential.
- Familiarity with NYS Entitlements, Housing, Health, and/or Mental Health/Substance Abuse Service Delivery System is an asset.
- Excellent interpersonal and verbal and written communication skills are required.
- Proficient computer skills, including Microsoft Office and Electronic Health Records, are necessary.
