What are the responsibilities and job description for the Youth Support Specialist position at Family Service League Inc.?
We are seeking a compassionate and dedicated Youth Support Specialist to join our team at Family Service League Inc. In this role, you will be responsible for providing support and guidance to young adults with mental illness and/or chronic medical conditions.
You will conduct home visits/meetings (both in-person and telehealth) with clients to develop strength assessments and individualized planning, provide linkages, assist with navigating service systems, and support them with their goal achievements.
As a Youth Support Specialist, you will work closely with our team of professionals, including Mental Health providers, Family Peer Support workers, and youth Peer Support workers, to ensure that our clients receive the best possible care.
