What are the responsibilities and job description for the Care Manager position at Family Service Society, Inc.?
Position Summary:
The Care Manager position is contracted through Family Service Society, Inc. and serves in the role of a Care Manager for the Steuben County Community Mental Health Center (SCCMHC).The Care Manager is to be the primary point of contact in managing children & youth services for a case. The Care Manager will be the center point of the communication to ensure all providers are working together with the child/youth’s best interest in mind. The Care Manager will conduct comprehensive assessments to ultimately craft a Plan of Care specific to the child. The Plan of Care will drive the scope and direction of the interventions. Case assignments made will require access to transportation to meet field work requirements made in the ordinary course of business in a timely and efficient manner. The work is performed under the direct supervision of a professional social worker on a consulting or direct supervisory basis, with leeway permitted for the exercise of independent judgment in developing and carrying out plans in individual cases. Furthermore, the work is carried out in accordance with standard accepted social work methods as they are applied to semi-professional tasks, as well as, the local policies and procedures as developed by SCCMHC and Children’s Health Home of Upstate New York (CHHUNY).
Duties:
- Responsible for, but not limited to complete CANS assessment, safety plan, outreach and engagement, service and treatment linkage and coordination
- Coordination of care with community service providers that are part of the client care team including periodic meetings to ensure progress.
- Assessment of psychosocial supports and linkages with medical, dental and behavioral health care providers, as well as, educational, employment, transportation community resources
- Provides coordination of services between all the providers attached to the case, for clients with complex psychiatric, substance use, and/or comorbid medical conditions.
- As needed, utilize electronic assisted devices and other interactive technology to communicate effectively with other providers (medical staff, addiction and mental health), as well as the child and family.
- Help improve, measure, monitor, and sustain quality outcomes that focus on clinical indicators/performance measures, patient satisfaction, and plan adherence
- Participate actively in team meetings and supervision to enhance professional development.
- Provide care management services to children/youth throughout Steuben County regardless of Medicaid status.
Education and Experience Requirements:
MINIMUM QUALIFICATIONS: Graduation from a regionally accredited or New York State registered college or university with a Bachelor's Degree which shall have included at least 30 credit hours in the social or behavioral sciences.
SPECIAL REQUIREMENT(S) Possession of a valid Class D New York State Driver License is required for appointment and must be maintained on a continuous basis throughout the term of appointment.
