Title: Section 8 Case Manager
Report to: Senior Director, Youth & Family Programs
Job Summary:
The Case Manager provides case management services to clients in the Section 8 program at Yonkers Municipal Housing.
Responsibilities:
1. Perform assessments for new referrals
2. Review initial resident assessments to identify the social, economic, and health care needs of the resident
3. Develop case management plans to include referrals to supportive services, governmental and private agencies, health care related facilities and other service providers to maximize the strengths, alleviate stressors, and increase self-sufficiency of residents
4. Conduct referrals in accordance with the case management plan
5. Complete telephonic contacts and home visits, as necessary, as well as for the purpose of updating and reassessing case management plan
6. Assist clients with Section 8 applications / recertifications as needed
7. Maintain and track case management services
8. Complete and submit reports
Requirements:
· Bilingual English/Spanish preferred
· Bachelor’s Degree in Social Work, Education or related field
· 1-2 years’ experience working with families and/or with housing
· Microsoft Office proficiency
· Valid driver’s license and use of a car
Salary: $25/hr, 18 hours a week
Job Type: Full-time
Pay: $25.00 per hour
Expected hours: 18 per week
