Job Description: The Peer Specialist works in collaboration with the treatment team to help engage and assist individuals in successfully managing their recovery. The Peer Specialist can provide both individual and group sessions that are strength based and empower the individual. Applicants will provide on-site and off-site services to individuals for PROS and CORE.
Requirements: Applicants are expected to self-identify as someone who has received current or past mental health services and verbalizes a commitment to their recovery. They must have at least a high school diploma or GED. The individual should possess a New York State Peer Academy Certification. They must have reliable transportation.
The ideal candidate will work well with others, be committed to their own recovery, be willing to share their store, be strength-based, and be empowering to others. In addition, will have prior experience in a behavioral health or rehabilitative setting. Be a compassionate listener and dedicated to supporting the personal growth needs of their clientele. In addition, they will have good written and oral communication skills and will be organized, with ability to manage multiple tasks and deadlines concurrently.
Responsibilities:
- Provide on-site and off-site evidence-based interventions and treatment to a reduced caseload of PROS individuals.
- Deliver community-based services to CORE enrolled individuals.
- Attend ongoing training related to PROS, cultural competency, risk assessment, and evidenced based practice.
- Complete timely documentation as required by NYS OMH regulations, including but not limited to: initial screening note, comprehensive assessment, service specific assessment, IRPs, ISPs, monthly notes, and progress notes.
- Maintain accurate, thorough records on all assigned cases and use documentation to assist individuals in evaluating progress.
- Inform supervisor, and other members of treatment team as indicated, of issues/concerns related to individuals.
- Participate in outreach, engagement, and discharge planning, case coordination and management and, as needed, crisis intervention.
- Assist individuals in increasing their overall functioning through added support and coping skills; assist individuals in identifying their strengths and barriers.
- Works collaboratively with staff and other community agencies on behalf of individuals and/or the agency.
- Provide follow-up care and make referrals to community service providers as appropriate.
- Participates, engages, listens, and learns in individual and team meetings and supervisions.
- Participates in required training to maintain certification.
- Responsible for reviewing and adhering to all agency policies and procedures.
- Supports a culture that prioritizes professionalism, kindness, teamwork, staff development, and compassion to ourselves, each other, and our individuals.
- Responsible to represent the agency in the community with courtesy and professionalism.
- Other assignments and tasks may be added at the discretion of their supervisor
Job Type: Full-time
Pay: $17.00 - $20.80 per hour
Expected hours: 37.5 per week
