Job Description: Family Services of Westchester is seeking full-time Community Wellness Specialist for our COMPHS Program
Position Responsibilities:
- Conduct depression and anxiety screening utilizing standardized screening tools
- Provides brief counseling, education, and resources for support services including referrals for mental health services
- Assess individuals' needs related to Social Determinants of Health and proved resources
- Conducts in-person and digital/remote outreach (phone, text, and virtual) to engage with community members
- Attends/tables at community events such as Heritage festivals, walks/runs and other local events to connect with the community and promote programs and services
Requirements:
- Bachelor's Degree in related field
- Bilingual English/Spanish preferred
- Driver's license
