CASE MANAGER-SAFE HOUSE
JOB DESCRIPTION
Status: Full Time / Non-Exempt
Hours: Monday - Friday – 3:00pm-11:30pm
Reports to: Safe House Manager
POSITION SUMMARY: Partners with clients to develop and monitor case plans. Case Manager provides individual and group sessions. Develops a network of resources throughout the community to enhance the ability to be a liaison for clients.
DUTIES AND RESPONSIBILITIES:
- Responsible for implementing all programmatic and agency protocols as directed.
- Develops, implements, and monitors case plans with all clients.
- Weekly client meetings to help achieve desired outcomes.
- Consistently reviews and updates clients’ safety plans.
- Responsible for ensuring clients successfully navigate the social systems in order to get the services they are requesting and helping remove any barriers.
- Assists clients in understanding the court proceedings and helps them prepare for what to expect.
- Provides support and guidance to clients to ensure orders of protection are obtained and facilitates access to additional legal support when necessary.
- Provides referrals and resources to clients
- Facilitates support groups and house meetings under the direction of the Director of Advocacy Services.
- Upholds all laws, policies, and procedures of Family Support Services, the State of Texas, City, County, and Federal regulations.
- Attend all meetings required.
- Maintains confidentiality of Safe House site and clients.
- Appropriately maintains accurate information in Osnium and entered in a timely manner.
- Other related duties as required.
REQUIRED COMPETENCIES:

- Knowledge of dynamics of domestic violence, family violence, sexual assault, and human trafficking.

Crisis Intervention Techniques
- Effectively demonstrates de-escalation of agitated clients.
- Assesses potential crisis situations and takes appropriate action.
- Reacts timely and logically in crisis.
- Responds appropriately to callers on the crisis line.
Case Management
- Demonstrates knowledge of community resources.
- Demonstrates competency in forming relationships and coordinating with other agencies.
- Demonstrates respect and caring to clients.
- Formulates appropriate referrals.
Documentation
- Completes all documentation thoroughly, accurately, and in a timely manner.
- Develops, implements, updates, and monitors progress on goals and objectives on individual case plans in a timely manner.

QUALIFICATIONS:
- A minimum of a bachelor’s degree in human services field (counseling, social work, psychology, sociology, or other behavioral health science) an equivalent combination of experience and education may substitute for education.
- Valid Texas Driver’s License and proof of auto liability insurance.
- Clean criminal background check.
- Proficient computer skills using MS Programs. Database knowledge helpful.
PHYSICAL REQUIREMENTS:
- Ability to sit at a desk and perform routine paperwork
- Ability to communicate by telephone
- Ability to travel between assigned work sites by automobile
- Ability to lift 25 pounds
