Job Summary:
The Aftercare Reengagement Specialist is responsible for supporting the planning, preparation and development of skills for youth who have been in the foster care system. This position will work in partnership with former foster care youth to establish and accomplish goals aimed at preparing them to be self-sufficient. This position will also work with disconnected youth ages 18-23 to provide emergency or one time assistance, as needed.
Job Duties:
- Conduct My Pathways to Success staffings with disconnected foster youth to assist with making a plan for self-sufficiency.
- Conduct aftercare plans and assist in completion of tasks as based on an assessment of needs and identified goals for eligible youth adults ages 18-23.
- Conduct life skills training as needed.
- Develop a life skills training plan based on the results of the Independent Living assessment completed by the young adult. This will include but not limited to life skills classes.
- Facilitate and coordinate individual and group life skills training events and activities for current and former foster care youth.
- Establish and maintain community partnerships that will assist with providing support and resources to former foster care youth in the areas of housing, employment, education, physical/mental health, parenting, transportation, nutrition/food and life skills.
- Participate in My Pathway to Success transition staffings.
- Provide targeted services for 1823-year-olds who are receiving services through the PESS program and Aftercare.
- Maintain program documentation on assessments, trainings and services specific to the youth in FSFN within required timelines.
Minimum Qualifications:
Education: Bachelors degree in the Human Services field or at least 3 years of professional experience providing services to teens and young adults.
Experience: At least three years experience coordinating projects, working with community partners and managing staff. Minimum three years experience working in the child welfare system work with high-risk youth. Experience working with youth and young adults preferred.
Licensure, Certification, and/or Registration: Valid drivers license. Child Welfare Certification preferred but not required.

Other Requirements:
- Clear a Level II background screening
- Clear a reference check
- Clear a local background check from the County in which you reside
- Clear a substance abuse screen
- Provide a copy of all degrees prior to your first day of employment
- Clear a Motor Vehicles Records check and must be insurable under the companys automobile policy.
- Provide copy of automobile insurance.
- Provide proof of and maintain a minimum personal auto liability insurance level $100,000/$300,000 if deemed to be a driving risk.
