What are the responsibilities and job description for the Caregiver: Santa Monica Saturdays and Sundays 10 am to 8 pm position at Family Ties Home Care LLC?
Introduction:
Family Ties Home Care has a client in Santa Monica for a man who is 82 years old. He had cancer and has a heart condition. No dementia-- he is highly intelligent. He is independent but a fall risk and requires stand-by assistance. Meal prep is also very important along with driving him to his appointments. Hi is diabetic, and overall weak. The shifts are 10 am to 8 pm. A shift during the week is possible. The pay rate is $20.00 to $22.00 per hour plus applicable overtime.
Must have Caregiving experience, a Driver's license, and a Car.

Responsibilities:
- Following a pre-set care plan
- Standard ADLs (personal care, bathing, dressing, grooming, hygiene assistance)
- Assist with medication reminders
- Monitor and report on client's well-being
- Light housekeeping, laundry, & other household daily chores as needed
- Grocery shopping & errands
- Cooking/meal preparation
- Scheduling appointments, including doctor's
- Communication with Family Ties HC management & family members/loved-ones regarding the client's daily status
- Daily notes
- Other duties assigned as needed
Qualifications:
- Current HCA or higher certification
- Caregiving experience of 1 year or similar experience
- Compassionate and caring demeanor
- Excellent communication & interpersonal skills
- Ability to follow the pre-set care plan and medication schedule
- Ability to lift 35 pounds
- Ability to assist with client's mobility as needed
- Valid driver's license and reliable transportation
- Ability to work the schedule as described
