Job Overview :
As a vital member of our healthcare team, you will provide in-home care for our clients with warmth and professionalism. We offer flexible schedule options for full-time, part-time, and PRN positions, and a supportive work environment.
Key Responsibilities :
- Travel to patients' homes and manage their care plans as directed by physicians.
- Assess and document observations of patients' conditions during each visit.
- Perform evaluations, including medication reviews and vital signs monitoring.
- Dress or redress wounds, checking on healing progress.
- Educate caregivers and families on ongoing patient care.
- Ensure patient comfort through palliative care measures.
- Recommend tools and devices to enhance patients' quality of life.
