Social Worker (Part-Time, 20 hours a week)
The FamilyMeans’ Caregiving and Aging program is currently seeking an part-time Social Worker! The purpose of this position is to provide assessment, support, and resource referral services to family and friend caregivers of persons of any age and assist individuals, families, and the community through challenges that can accompany aging. This position is fundamental to the Caregiving & Aging program’s success as this individual assesses the variety of caregiver circumstances presented, offers and implements a spectrum of services, balances the immediacy of family needs with the availability of appropriate support, and manages transitions between in-person and virtual programming as environmental circumstances dictate.
This role is an hourly, nonexempt position that would work 20 hours per week. Primary location for this position would include the agency’s Stillwater, MN office.
Essential Duties and Responsibilities: % of Total Time
Provide Individual Caregiver Coaching and Consultation Using Evidence-Based and Other Best Practice Models (55%)
- Assess the needs of family or friend caregivers using established tools and protocols to understand their strengths and challenges in caring for someone who needs them.
- Develop, implement and evaluate plans to provide support for caregivers.
- Recommend type, frequency, intensity and duration of support services to address each caregiver’s particular needs.
- Observe and discuss outcomes of support provided and modify plan as needed.
- Provide information and referral regarding agency and community resources.
Facilitate Caregiver Support Groups and Lead Caregiver Education Classes (15%)
- Develop curriculum on an as needed basis to present to Support Groups including the development of topics for discussions.
- Utilize current curriculum, and update current curriculum as needed based on current best practices in Caregiving & Aging, while facilitating Education Classes.
I*nitiate and Facilitate Respite Services (5%)*
- Collaborate with volunteer coordinator to help ensure successful respite matches.
- Serve as family advocate in volunteer respite process.
Work to expand relationships with a broad spectrum of community members (5%)
- Nurture reciprocal connections with other organizations that serve diverse communities.
- Develop and maintain awareness of unique needs of various cultural groups.
- Participate with the team in adapting services to meet emerging needs.
- Represent FamilyMeans in professional networking, collaborative groups, and state coalitions.
- Monitor the environment for changes in need, identify trends, and anticipate how we may need to shift to remain current and relevant.
Administrative (10%)
- Maintain records of services provided, documenting client activity in all relevant databases in a timely fashion.
- Assist with Caregiving and Aging grant process.
- Assist in writing grant applications.
- Gather statistics regarding what and why.
- Evaluate outcomes and report to funders.
- Prepare quarterly statistical figures and necessary reports for funding sources.
Keep Abreast of Caregiving and Aging Best Practices (5%)
- Participate in new Caregiving and Aging program planning and development including adapting programs to accommodate virtual delivery methods as needed.
- Maintain level of expertise in individual work and as a leader in the landscape of peers.
- Function effectively as a team member in the many collaborative relationships relating to caregiver needs.
- Represent agency Caregiving and Aging interests and expertise to collaborators at local, state and other meetings.

- As with all FamilyMeans staff positions, this position is responsible for helping to ensure volunteers and interns have an exemplary experience during all interactions with FamilyMeans and is expected to consistently maintain a high level of customer service to all guests, clients, and fellow team members.
- Assist in the administrative support and planning of education programs and special events as needed.

Minimum Qualifications
Education, Licenses and Certifications
Bachelor’s degree in social work is required; a master’s degree in social work is preferred.
An active Minnesota social work license is required.
Minnesota Caregiver Coaching, Powerful Tools for Caregivers, and REACH training preferred.
Experience
- Minimum of 5 years’ experience working in the Caregiving & Aging field
- Combined experience working with children and adults with disabilities, older adults in need of care, and their families is preferred.
- Previous Caregiver Coaching and Consultation experience is preferred.
- Previous experience in facilitating groups is preferred.
- Previous experience working with, and guiding volunteers is preferred.
Knowledge and Skills
- Strong interpersonal and proven team skills.
- Excellent assessment skills.
- Excellent oral and written communication skills. Requires the ability to effectively present information and respond to questions from clients, employees, managers and the public.
- Excellent organizational and documentation skills.
- Knowledge of the dynamics of caregiving.
- Ability to work with and maintain confidential information.
- Familiarity with Microsoft Office tools, web-based client data management, and videoconferencing platforms.
- Valid driver’s license with a driving record that meets agency qualifications, reliable vehicle, and necessary insurance.
- Commitment to work collaboratively with all staff, volunteers, program participants, and other supporters.
- Ability to relate effectively and in a positive manner with people from diverse social, cultural, economic and educational backgrounds.
- Willingness to develop cultural competence related to the services provided by the agency.
Physical and Mental Demands:
The mental and physical requirements described here are representative of those that must be met by an individual to successfully perform the essential functions of this position.
While most job duties will be located in office, this position requires the ability to drive within the service area, to interview caregivers and provide needed support. Must have the ability to freely move between and around locations. The incumbent must be able to read and write extensively, to hear, to speak and to see. While performing the duties of this position, the employee is frequently required to use hands to handle, grip or touch objects, tools or controls; occasionally lift, move and/or carry up to 20 pounds; and at times, sit for extended periods of up to several hours. Individuals must be able and willing to do this work on the phone or on the screen, providing services virtually as needed. Equipment used includes, but is not limited to, computers, automobile, telephone, fax machine and copier.
Mental demands of this position include comprehension, reasoning (including the ability to combine pieces of information to form a conclusion), and decision-making. Requires the ability to maintain sensitivity to employee and client diversity and demonstrate respectful treatment of others. Must be adaptable and respond effectively to changes in situations or information.
Working Environment:
The work environment characteristics described here are representative of those individual encounters while performing the essential functions of this position.
The working environment is a combination of commuting while driving within the service area to interview caregivers and provide needed support, office (onsite) work, and remote work as needed. The environment also includes virtual platforms when providing virtual/remote services as needed.
Schedule: We are open to schedule as long as core hours are Monday - Friday 8am-4:30pm (may vary occasionally due to client needs)
Pay: Hourly, range $26.02-29.92 (This position is considered hourly due to not meeting the salary threshold requirements due to its part-time status)
