What are the responsibilities and job description for the High School Special Education Case Manager position at Farmington School District?
- Plans and/or monitors a program of study that meets the individual needs, interests, abilities, and goals of students.
- Creates a classroom environment that is conducive to learning and appropriate to the maturity and interestes of students.
- Guides the learning process toward the achievement of curriculum goals. Clear objectives that support State Standards will be established and communicated for all lessons, units, projects, and related activities.
- Employs instructional methods and materials that are most appropriate for meeting the stated goals and lesson objectives.
- Asses the accompishments of students using a variety of methods on a regular basis and provide progress reports as required.
- Consults with district specialists regarding students exhibiting learning difficulties and participate in the referral process for students with suspected learning disabilities.
- Complies with federal regulations regarding students with special needs and maintains such records as required by law and district policy.
- Communicates with administration, parents and colleagues on a regular basis to ensure progress toward student IEP goals.
- Assists the administration in implementing all policies and procedures, including the development of reasonable rules for behavior.
- Supervises students in hallways before, after and in between classes, library and special events.
- Attends staff meetings and serves on staff committees as required.
- Plans and oversees purposeful assignments for paraprofessionals in consultation with appropriate staff ( classroom teacher, support personnel, and administration).
- Strives to maintain and improve professional competence.
- To discharge such other duties and responsibilities as assigned by the administration.
- Assists parents with written referral requests.
- Preparation of IEPs.
- Participation in IEP, manifestation, and review meetings as necessary.
- Provides direct instructional services to students on caseload.
- Reviews and recommends accommodations and modifications to academic programming as appropriate for the student along with regular consultation with classroom teachers.
- Communicates with parents and the IEP team in a timely and professional manner regarding meetings, services, and progress.
- Updates Easy Medicaid grid for billable Medicaid services.
- Maintains district records for all student services provided to students on caseload.
- Ensures timely notifica5tion for all steps in the referral and IEP process, following all procedural safeguards.
- Transition planning for students as appropriate.
- Remains up to date on statutes pertaining to timelines and provision of student services.
- Maintains confidentiality of student records at all times.
- Implements DLM.
- Physically able to perform the essential function of the job as outlined above, in addition to the following.
- While performing the duties of this job, the employee is frequently required to stand, walk, and sit.
- The employee is regularly required to see, talk, and hear.
- The employee is regularly required to use hands to manipulate, handle and feel.
- The employee is occasionally required to reach with hands and arms, climb steps, stoop, kneel, or crouch.
