The Full time Care Coordination Specialist will play a pivotal role in implementing evidence based strategies, fostering collaboration, collecting, and analyzing data, and actively participating in various meetings to contribute to the success of substance misuse prevention and mental
health promotion initiatives in Oswego County, specifically for the Strategic Prevention Framework (SPF)-Partnerships for Success for Communities grant funded project. The Care Coordination Specialist will carry out the following responsibilities:
Essential Job Duties and Responsibilities: (Other duties as assigned)
Utilize the Strategic Prevention Framework (SPF): Implement the Strategic Prevention Framework to strategically plan and execute initiatives aimed at addressing up to three community substance misuse prevention/mental health promotion priorities based on data analysis.
Collaborate with Community Partner: Foster collaboration with project partners that serve the identified population in Oswego County. Work closely with relevant organizations and stakeholders to enhance the impact of prevention efforts, including ARISE and the Oswego County Office for the Aging.
Provide Evidenced-Based Programming: Deliver evidence-based programming tailored to address substance misuse prevention and mental health promotion. Implement interventions grounded in proven methodologies and strategies. This includes the WISE program, SBIRT (Screening, Brief Intervention, Referral to Treatment), and Contingency Management.
Reduce the incidence of medication thefts and prescription misuse: Provide Deterra bag distribution, complete installation of drug-disposal Dropboxes that are convenient access for individuals over 60 and provide free medication lockboxes to secure potentially dangerous and addictive drugs and prevent unauthorized access.
Social Media: utilize multi-media platforms to launch a medication awareness campaign that covers storage, disposal, Deterra bag distribution, and reporting medication theft
Collect Data: Systematically gather and document relevant data to assess the effectiveness of prevention initiatives. Monitor and evaluate outcomes to inform decision-making and strategy adjustments.
Project Performance Assessment: Conduct assessments of project performance to ensure alignment with established goals and objectives. Identify areas for improvement and implement corrective measures as needed.
Complete GPRA's (Government Performance and Results Act): Fulfill requirements related to Government Performance and Results Act reporting. Provide accurate and timely information on project outcomes and progress.
Actively Participate in Project Meetings: Engage proactively in project meetings, contributing insights, updates, and collaborating with team members to ensure effective communication and coordination.
Engage in Prevention Advisory Committee Meetings: Actively participate in Prevention Advisory Committee meetings. Contribute to discussions, share expertise, and collaborate with committee members to enhance the overall effectiveness of prevention strategies.
Policies and Procedures: Adheres to all applicable local, state, federal and agency regulations, policies, and procedures.
Communication: Assists with the implementation of Grants and helps maintain effective communication between all parties responsible for oversight of these initiatives.
Knowledge, Skills, and Abilities
Must demonstrate experience in substance prevention programming.
Poses a proven track record in designing, implementing, or coordinating substance use prevention initiatives, and exhibit knowledge in Evidence-Based Programming.
Must be organized, proficient in computer skills including word processing and data entry and have strong communication skills.
Flexible hours, which can include some evening and weekend delivery of services.
Valid drivers license and reliable transportation to ensure the delivery of services throughout Oswego County and collaboration with community partners.
Requires flexibility, initiative, integrity, excellent interpersonal skills, project coordination, ability to work well with all levels of internal management and staff, as well as outside clients and vendors, attention to detail, understanding of agency, state, and
federal regulations.
Education and Experience:
Bachelor's degree in a relevant required.
Experience working in SUD prevention programming is essential
2 years serving the population of focus.
Familiarity with Oswego Counties cultural and language.
Experience in working on grant-funded projects is preferred.
Proven track record in coordinating SUD prevention initiatives and knowledge of EBPs.
