Fast Pace Health is seeking a highly skilled and experienced Primary Care Nurse Practitioner or Physician Assistant to join our growing team. As a Primary Care Provider, you will play a critical role in delivering high-quality healthcare services that prioritize patient-centered care and community engagement.
Job Summary:
The Primary Care Nurse Practitioner or Physician Assistant will be responsible for providing comprehensive primary care to patients, focusing on proactive health management and preventive care. This role emphasizes patient-centered care and involves working collaboratively with healthcare teams to enhance health outcomes in rural communities.
Responsibilities:
- Conduct thorough physical examinations and assessments for patients of all ages, including children ages 3 years and up.
- Perform chronic care management for patients with conditions such as hypertension, diabetes, and asthma.
- Administer annual wellness visits for adults and pediatrics, including vaccinations and preventive screenings.
- Order, interpret, and evaluate diagnostic tests to diagnose and assess patients' clinical problems and healthcare needs.
Requirements:
- Nurse Practitioner (NP) / Physician Assistant (PA) with 2 years of dedicated experience in a direct primary care setting
- Proficient in providing comprehensive primary care, including chronic care management, annual wellness visits for adults and pediatrics, and preventive health measures
- Strong understanding of Patient-Centered Medical Home (PCMH) principles and value-based care
- Willing to perform pediatric exams and see children ages 3 years and up
