Job Description
The Primary Care Provider will treat patients and provide quality healthcare through direct contact and mediated communication. They will interact closely with clinic members, including telehealth team members, other Primary Care Providers, and telehealth call center team members, to achieve excellent patient care.
- Conduct thorough physical examinations and assessments for patients of all ages
- Perform chronic care management for patients with conditions such as hypertension, diabetes, and asthma
- Administer annual wellness visits for adults and pediatrics, including vaccinations and preventive screenings
