The role of a Primary Care Nurse Practitioner or Physician Assistant involves providing comprehensive healthcare services in a friendly and encouraging environment.

Key Responsibilities:

• Conduct thorough physical examinations and assessments for patients of all ages, including children aged 3 years and above.
• Manage chronic care for patients with conditions such as hypertension, diabetes, and asthma.
• Administer annual wellness visits for adults and pediatrics, including vaccinations and preventive screenings.
• Order, interpret, and evaluate diagnostic tests to diagnose and assess patients' clinical problems and healthcare needs.

Collaboration and Communication:

• Collaborate with physicians and other healthcare professionals to develop comprehensive patient care plans.
• Promote patient engagement, health literacy, and shared decision-making.
• Participate in quality improvement initiatives and value-based care programs.

Requirements and Preferences:

• Nurse Practitioner (NP) / Physician Assistant (PA) with 2 years of dedicated experience in a direct primary care setting.
• Proficient in providing comprehensive primary care, including chronic care management, annual wellness visits for adults and pediatrics, and preventive health measures.
