Position Title: DOJ - Case Manager
Reports To: Director, Coordinator
Status: Full-Time / Non-Exempt
Direct Reports: N/A
Description of this role: Responsible for assessing and assisting fathers participating in the Fathers and Families Support Center programs. Provide services to re-entering offenders during pre and post release.
Duties, Functions and Responsibilities - includes the following and other related duties may be assigned.
  • Department of Justice project working with the re-entry population and ex-offenders.
  • Provide case management services to re-entering offenders’ pre- and post-release.
  • Completes a social assessment, which includes personal interviews as well as collection of secondary information to complete an individualized plan for self-sufficiency and responsible fatherhood.
  • Assists client in overcoming barriers through intervention, advocacy, and referrals.
  • With the support of the individual, identifies achievable goals, barriers, provides personal support and counsel to participants achieving identified goals.
  • Facilitates peer and family support groups and classes, including the development and selection of topics and programs and securing outside guest speakers.
  • Completes drug screening, police, and abuse records check.
  • On-going case management to develop case plans to address offenders' needs.
  • Documents individual records and forms, as necessary, to provide accurate and timely tracking.
  • Develops and maintains network of community resources and social support systems, as necessary, to assist individuals and their children/families to move to self-sufficiency.
  • Review and possess a working knowledge of all programs, forms, and grants related to your duties.
  • Must be able to work flexible schedule, some evenings, and may be required to assist with 6 Saturday events per year. Mandatory to attend all staff and department meetings, client graduations, and bonding events.
Knowledge, Skills and Abilities:
  • Must be culturally competent in working with men who need maximum direction regarding responsible parenting issues.
  • Must be able to identify and apply cognitive behavioral interventions.
  • Must have effective verbal and written communications skills and strong interpersonal skills.
  • Must be able to utilize agency and community resources to meet goals.
  • Must be able to organize and present information
  • Must be able to multitask, be team oriented, and be able to work independently.
  • Must understand and practice the NASW code of ethics and be familiar with MS Office Suite.
Educational and Experience Requirements:
  • B.S. in Human Services or related minimum; or BSW, MSW preferred.
  • Must have experience in working with at-risk populations, group facilitation, individual and family counseling, re-entry, and offender population.
  • Must have valid Missouri Driver's license and reliable, insured vehicle.
