This Hospice Director of Case Management / RN will be responsible for the following duties :
- Oversee the daily operation of the Case Management Department.
- Coordinate hospital wide functions of Case Management, Utilization Review and Discharge Planning.
- Oversee and coordinate the collection of pertinent Hospital data.
