Job Summary
We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Home Health team. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. This role requires a strong background in patient assessment and the ability to work independently while maintaining effective communication with patients, families, and healthcare teams.
Duties
- Conduct comprehensive patient assessments to determine health needs and develop individualized care plans.
- Administer medications and treatments as prescribed, including ostomy care and sterile processing.
- Provide health coaching and education to patients and their families regarding disease management, medication adherence, and lifestyle modifications.
- Monitor patient progress and adjust care plans as necessary based on ongoing assessments.
- Collaborate with interdisciplinary teams to coordinate care for patients with complex needs, including those requiring trauma medicine or memory care.
- Maintain accurate documentation of patient care activities, assessments, and outcomes in compliance with regulatory standards.
- Ensure a safe environment for patients by recognizing potential hazards and implementing appropriate interventions.
Experience
- Current RN licensure in the state of practice.
- Previous experience in home health or a related field is preferred but not required.
- Strong skills in patient assessment, trauma medicine, PICU experience, and knowledge of anatomy systems are highly desirable.
- Familiarity with health coaching principles and best practices for managing chronic conditions is beneficial.
- Experience with ostomy care and sterile processing is a plus.
