Normal Work Hours : Part time PRN.
Necessities :
Must keep the identity and health information of all clients confidential at all times.
Must maintain valid driver's license, registration, and vehicle insurance and be able to drive his / her vehicle for any work assignments.
Must follow DCFS mandated reporting requirements.
Must successfully pass a fingerprint and background check.
Must understand and speak fluent English.
Physical Demands of the Job :
Must be able to bend and stoop repetitively.
Must be able to be on ones' feet for prolonged periods of time.
Must have manual dexterity and control of hands in order to provide immunizations, blood draws, IVs, etc.
Must be able to lift required nursing equipment which is normally 20-40 pounds.
Must be able to work at a computer station for prolonged periods of time.
Must be mobile enough to fulfill the job requirements which include the ability to get in and out of the vehicle, traverse rough terrains, and enter spaces which may be totally handicapped inaccessible.
Environmental Factors :
Must be able to work in confined spaces as needed.
Must be able to work in conditions of extreme cold or heat as needed.
Essential Job Functions (Duties and Responsibilities) :
Provides professional nursing services according to physician's orders in conformance with recognized nursing techniques and procedures, established standards, and administrative policies of the local public health department.
Complies with accepted professional standards and principles.
Assesses and evaluates patient's status by reviewing and implementing the plan of care, regularly evaluating patient needs and revising the plan of care as necessary.
Administers medications and treatments as prescribed by physician.
Confers with patients, caregivers, physicians and therapists to gather pertinent information related to assignments.
Maintains and initiates clinical records by recording all pertinent information regarding the condition of the patient following each home visit. Completes charting in a timely manner (within 48 hours)
Teaches patient and family self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families.
Assists the patient's family and other health care personnel in providing continuity of care.
Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
Participate in an on call rotation
Performs all other duties as assigned.
Education and Experience Requirements :
Requires graduation from a school of nursing approved by the Department of Professional Regulation; and an Illinois license as a Registered Nurse by examination or Illinois license as a Licensed Practical Nurse; and one (1) year experience in a home care agency or hospital (preferred).
Possesses and maintains current CPR certification.
Possess and maintain yearly Alzheimer's Dementia training
