FCP Live-In is hiring a Field Supervisor

Position Summary

The Field Supervisor will work collaboratively with the Executive Director of Client and Caregiver Services, senior management, administrative teams, staff and outside stakeholders to ensure that the highest quality of home health care services are being provided to FCP Live-In’s clients.

Essential Functions & Responsibilities

  • Conduct regular field visits to client homes in order to maintain high standards of care for all clients in accordance with FCP Live-In standards of care and local, state and federal guidelines.
  • Assist in the development, implementation and oversight of care plan compliance.
  • Manage environmental management and safety including but not limited to oversight of staff care of client environment, cleaning client care area, handling of blood/body fluid spills.
  • Communicate with medical personnel, clients and family members regarding client health records, updates, changes in condition.
  • Assist in training staff and oversee the recognition of and reporting signs and symptoms of infection (abdominal pain, nausea, vomiting blood, urine/stool and difficulty urinating and/or diarrhea); endocrine problems (drowsiness; thirst and/or sweating).
  • Assist in maintaining informal and formal communication, provide written communication when required, be responsible for documenting actions by completing forms, reports, logs, and records as may be required.
  • Oversee staff’s ability to operate the following equipment and assistive devices which include but may not be limited to: walker, cane, crutches, wheelchair, commode, hospital bed, Hoyer lift, prosthetic devices, orthotic devices, hearing aids, assistive learning devices, augmentative and alternative communication devices, alerting devices, cognitive aids, stair lifts, lift chairs, gait trainers, household appliances.
  • Train staff when necessary and document successful completion of training.
  • Ensure that staff will uphold and comply with all safety programs and policies to maintain a safe environment for the client.
  • Advocate on behalf of the client and promote and protect the rights and dignity of each person. Monitor support relationships for potential abusive or neglectful situations.
  • Protect FCP Live-In’s value by keeping client information confidential, assist in ensuring agency records are maintained in a safe and secure area with specific access availability to ensure confidentiality, and ensure that agency records, files, documents and reports are the exclusive property of FCP Live-In.
  • Assist in overseeing staff participation and attendance in all agency mandatory training sessions and in-service training, as indicated by client needs and local licensing authorities.

Working Conditions/Hours

Combination of field, home of the client, office and other locations where the client may be receiving services, in the community, etc. The working hours are variable and flexible, depending upon the needs of staff and clients.

Education, Experience & Specialized Skills

COTA certificate and at least five years of relevant work experience, including experience in healthcare management. Must read, write and speak English. Experience in home healthcare industry preferred.

Salary : $60,000 - $62,400
