ESSENTIAL DUTIES, FUNCTIONS AND RESPONSIBILITIES
Works effectively within a multidisciplinary team (hospital wide) that includes physicians, psychiatrists, psychologists, advanced practice nurses, social workers and other allied health professionals.Works effectively within an interdisciplinary team that includes, but not limited to : the Outpatient Behavioral Health services, Adolescent Care Unit, Primary Care Clinic, Intensive Care / Multi-Service Unit, OB / GYN (INPT / OPT), and the emergency medicine services.Evaluates / screens psychological, social, physiological, and behavioral signs and symptoms of distress. Identify any coexisting conditions (medical, psychiatric, environmental, etc.) including risk assessment that indicates the need for additional professional assessment and / or services. Determine the patient's level of care needs and be knowledgeable about making appropriate referrals including safety planning.Performs the behavioral health intake process that includes completing intake documents, identifying the patient's presenting problem(s), and making a case presentation to the treatment team meeting for matching the appropriate provider with the identified patient.Provider should be knowledgeable about completing documents that include consent for treatment, patient's rights and responsibilities, confidentiality agreements, and consent to release information forms.Collaborates information from significant secondary sources regarding patient's development and psycho-social history.Obtains a mental status examination, assesses for risk to self and / or others, and be able to assess for a substance use disorder.Performs appropriate psychological and / or neuropsychological testing / assessment, biopsychosocial-cultural assessment, behavioral and developmental assessment, a needs assessment, including but not limited to, intergenerational and immediate family difficulties, chronic health problems, and behavioral / mental health problems.Explains assessment results in an understandable manner, identify and rank problems, establish agreement on immediate and long-term goals, identify effective therapeutic interventions and establish agreement on the treatment process and the resources to be utilized.Provides gender and culturally sensitive individual, group, and family focused behavioral therapy / counseling or psychotherapy.Coordinates and integrate services, agencies, resources, and people together within a planned framework of action toward the achievement of established treatment goals.Recognizes elements of a crisis, able to implement an immediate course of action, and appropriate treatment / intervention to address the crisis.Provides services as part of the behavioral health outpatient department after hours, onu0002call rotation schedule where applicable.Disseminates psycho-education to patients and their sources of support pertaining to the patient's strengths and weaknesses. Provides hospital and community resources to patients and their sources of support.Keeps abreast of new concepts and techniques in specialty areas and apply them as appropriate to the direct provision of care.Continues pursuit and development of the specialty areas for behavioral health programs and services enhancement.Documents in the Electronic Health Record (EHR) system direct and in-direct patient care that was provided (within 24-hours of service). This includes but is not limited to : assessments and treatment plan, reports, progress notes, case management, consultations, and other patient-related information. Records should be relevant to the patient's continuum of care.Consults with other professionals in and out of FDIHB health care network regarding a patient's comprehensive care and regarding a patient's continuum of care. Provider should be cognizant of their professional skill limitations and seek consultation accordingly.Adheres to applicable laws, regulations and agency policies governing the disclosure of patient identifying information.Works under the general supervision of the Chief of Behavioral Health who outlines broad major objectives of the departments.Will work with full professional independence. Work is reviewed through rounding and periodic conference and reports relative to achievement of department goals and objectives.Performs other duties as assigned.MANDATORY MINIMUM QUALIFICATIONS
Experience :
One (1) year of direct work experience as a clinical psychologist.
Education :
Doctorate degree (PhD or PsyD) in Psychology from an accredited institution.
Active, unrestricted Psychology license from any U.S. State or Territory.
