Description
The Residential Case Manager will provide supportive services to men and women in Residential substance use disorder/co-occurring treatment. The case manager works closely with the treatment team to accomplish the mission and service goals of Fellowship House. This position includes the following duties:
1. Ongoing assessment of programming’s strengths, limitations, and needs
regarding ASAM and ROSC fidelity
2. Work in a team oriented system, incorporating contributions of clients,
employees, family/collaterals and relevant community stakeholders
3. Utilize multiple streams of information to effectively and efficiently
develop and implement non-residential programs in pursuit of beneficial
residential client outcomes
4. Alcohol/drug testing of clients.
5. Support with progressive medication management, in support of case
managers and program assistants
6. Intake screenings, transition support, and groups/lectures for clients in
pretreatment, medium intensity residential, low-intensity and continuing
care programs.
7. Involvement in community outreach programs; i.e. in-services, community
outpatient clinics and workshops.
8. Maintain the residential waiting lists and adherence to specific external and
internal reporting mechanisms (Medics, ASAIS, private contracts, monthly
reports, etc.)
9. Maintain effective liaisons with diverse referral resources, locally and
statewide, to facilitate appropriate placements for individuals in need of
Pretreatment and residential services.
10. Remain compliant with grant standards and requirements.
Work schedule specific to this employee:
Holiday coverage in rotation with other staff;
Extended hours when needed to provide continue direct care service
Regular Schedule: Monday-Friday 8:00 AM- 4:00 PM
Requirements
-Bachelor's degree in behavioral health related field preferred, or high school diploma/GED with an ADC or CRSS certification.
-Completion of case management training.
-Valid driver’s license with good driving record.
-Must have and annually renew CPR certification, First Aid certification, and Medication Assistance Certification through Fellowship House Nurse Delegation Program.
-Knowledge of all Fellowship House’s levels of care, including mission, management, objectives and operations; should continually increase as part of ongoing professional development
-Knowledge of local resources serving the needs of clients (medical, clothing, etc.); should continually increase as part of ongoing professional development
-General knowledge of the bio-psychosocial effects of addiction/co-occurring disorder in the individual, family, and community; should continually increase as part of ongoing professional development
-Awareness of Recovery Oriented Systems of Care - Recovery Principles; knowledge of NIDA Principles of Effective Treatment; should continually increase knowledge as part of ongoing professional development
