We're seeking a Home Care Visiting RN Supervisor to join our non-medical Home Community Based Services team based at Fellowship Village in Basking Ridge, NJ. \n- Adhering to the Best Practices established by the Board of Nursing and CAHC Accreditation Standards
- Open communication with other Team Members, Clients, and Family
- Provide input regarding placing certified home health aides (CHHAs) appropriately with clients
- Supervision of CHHAs in the greater community
- Making monthly follow-up calls/visits to ensure adherence of Care Plan
- Evaluate effectiveness of care provided by CHHAs with timely completion of 60-day skills assessments during on-site visit
- New Jersey Registered Nurse (RN) License, BSN preferred.
- BLS Certification
- New Jersey Driver’s License, and an automobile in good working condition with appropriate insurance coverage in accordance with New Jersey laws.
- Minimum of one-year supervisory experience in health care setting, preferably home care, hospice, or long-term care
- Ability to join on-call rotation
- Exceptional communication and interpersonal skills
- Robust critical thinking and problem-solving abilities
- Capable of working independently and collaboratively in a multidisciplinary team
- Proficient in electronic medical record systems and relevant software applications
