Program Description
Bridge Housing is intended to address the immediate shelter and treatment needs of people who have serious behavioral health conditions and are experiencing homelessness. The site management and support services team are responsible in maintaining a 80-bed transitional recovery and community re-entry housing facility for those suffering from mental illness and / or substance use disorder who are experiencing homelessness. The target population often suffer from complex and co-occurring medical conditions and other characteristics that can challenge their ability to navigate mental health, physical health, and substance use treatment in the Santa Rosa community. This interim shelter is for individuals experiencing homelessness who have serious behavioral health conditions and / or substance use disorder issues who are willing to receive treatment. A particular focus will be Community Assistance, Recovery, and Empowerment (CARE) Court clients with a court-ordered care plan. CARE Court is aimed at helping those suffering from untreated mental health and substance use disorders that lead to homelessness, incarceration, or worse. CARE Court connects a person struggling with untreated mental illness - and often also substance use challenges - with a court-ordered Care Plan for up to 24 months.
Each plan is managed by a care team in the community and can include clinically prescribed, individualized interventions with several supportive services, medication, Bridge Housing Site Management staff will be responsible for the overall operations of including crisis intervention, behavioral health, substance use disorder and medication support and a housing plan. Services include 24 / 7 medical and clinical staffing with a department-assigned Housing Coordinator who will provide "gatekeeping" to assure appropriateness of all referrals. Referrals must be clients with a mental illness and / or co-occurring substance use disorder and are either precariously housed or are unsheltered. The proposed services will provide enhanced support to help homeless clients quickly exit to permanent housing utilizing a client-centered modality. A client-centered approach can also include a public defender and supporter to help make self-directed care decisions in addition to their full clinical team.
Position Summary
The Clinical Program Manager is a licensed clinician with certification to provide clinical supervision and will function primarily in supervisory position and some direct service as needed. The primary functions of this role include responsibility for implementation of the coordinated specialty care model program, clinical supervision, coordination of team activities, training, monitoring of quality assurance, and supporting fidelity of program models. Provide direct support to site management staff who offer integrated mental health and substance use support through individual and group interventions including Cognitive Behavioral Therapy, Dialectical Behavioral Therapy and targeted life skills. The Clinical Program Manager strives to create a welcoming, safe, supportive and culturally affirming environment and provide direct supervision and oversight to site management staff, e.g., Clinical Case Managers, Case Managers and Peer staff.
Program Manager Specific Responsibilities
- Review and approve clinical documentation ensuring charts and medical records meet Medi-Cal / Medicare / payor requirements, compliance requirements, and utilize client centered approach
- Provide individual and group supervision to staff accruing clinical experience and hours for professional licensure (according to licensure status).
- Support the multi-disciplinary team's compliance with all program protocols and best practices through consultation and multi-department cross-training and team building.
- Have proficient knowledge of (and / or able to receive training) Motivational Interviewing, Cognitive Behavioral Therapy, and other evidence-based and evidence-informed practices suitable guests.
- Develop and implement Wellness Care Plans with participants in collaboration with community partners and the court.
- Supervise the monitoring and tracking of guest's progress in achieving their goals and objectives.
- Develop and maintain strong clinical relationships with guests with an emphasis on clear boundaries.
- Ability to manage multiple databases, use the Office Suite and cloud-based file storage.
- Participate in weekly staff meetings, collaborative partner meetings and Care Court meetings.
- Adheres to laws regarding confidentiality and reporting requirements and maintains necessary compliance documentation.
- Other duties as required.
