Feonix - Mobility Rising is a global Mobility as a Service innovator that seeks to solve transportation challenges for underserved populations, including seniors, individuals with disabilities, caregivers, and families in poverty. Feonix accomplishes this goal by working with local organizational leaders and transportation options and connecting them into state-of-the-art technology to provide a one-stop shop for available mobility options.
The goal of the Community Resource Mobility Navigator (MN) is to help individuals source transportation as a step in accessing available social services and other community resources. The MN provides hands-on, intensive support to riders, transportation providers, and organization contacts to help them achieve success in navigating and budgeting for their transportation needs. This person also helps to keep track of data and pull reports that a Community Development Manager and the Mobility Leadership Circle use to inform the next steps in the program.
The MN reports to the Director of Mobility under the supervision of the lead Mobility Navigator, and works closely with the community development team, the Operations Manager, and the Community Operations Support Center team.
Primary Duties and Responsibilities
- Improve transportation access to programs and services within the community
- Identify community resources, answers questions about services and benefits
- Assist in developing an action plan, clearly detail steps and timelines to follow
- Assist with applications, appointment setting, referrals and connection to community resources and programs
- Attend appointments with clients as needed to help enable better communication
- Provide referrals to community agencies
- Act as an advocate
- Work with patients and clients with health-related social needs to leverage and coordinate community-based services to promote, improve and maintain health and to provide support for self-sufficiency
- Provide navigation to Medicare and Medicaid beneficiaries with identified health-related social needs
- Conduct analysis of needs and provide coordination of services. Develop person-centered, individualized referrals, customized action plans, and client advocacy
- Attend navigator meetings with community resource organizations to increase knowledge of services and allow for ‘warm transfers’ from clients to local services providers
- Collaborate with clients to ensure that they are meeting their goals and offer support to continue the path of self-sufficiency by providing encouragement and alternate resources and/or services.
- Gather data, analyze findings and apply logic and reason.
- Interpret, monitor, and report information and statistics.
- Compile and sort data and articulate issues and recommendations.
- Coordinate deadlines and prioritize competing demands.

- Assists with transportation referrals, authorizations and re-certifications
- Identifies appropriate community social service agencies and other resources to assist patients and families. This may include, but is not limited to, assisting patients in securing financial assistance obtaining medical transportation and any other public and private resources deemed necessary.

Education, Experience and Knowledge
- Associate’s degree in social work or related human services field
- 1 - 2 years of healthcare experience or related with specific knowledge of and experience with navigating community resources
- Knowledge of social service networks, medical community, and community-based resources
- Knowledge of federal, state, and local laws, codes, and regulations
- Familiar with Medicare and Medicaid
- Proficient with PCs and Microsoft Office applications.
