We are seeking an enthusiastic, NC Certified Peer Support Specialist. Fernandez Community Centers mission is to improve the quality of mental health and substance abuse services by reducing stigma and by protecting the rights of the individual who suffers from mental illness and/or substance abuse. General Description of Position: The Peer Support Specialist plays a crucial role in supporting the TCM team to deliver high-quality services to members. Peer Support Specialists provide valuable assistance in coordinating care, engaging with members, and ensuring seamless communication within the care team. They contribute significantly to the overall efficiency and effectiveness of Tailored Care Management.

Duties and Responsibilities:

  • Member Engagement: Reach out to members, establish rapport, and assist in gathering necessary information for care planning.
  • Care Coordination: Assist in coordinating appointments, services, and transportation for members. Ensure members receive necessary reminders and follow-ups.
  • Data Management: Maintain accurate and up-to-date records of member interactions, appointments, and services provided.
  • Team Collaboration: Work closely with Care Managers and other team members to ensure seamless communication and efficient care delivery.
  • Resource Assistance: Provide information and assistance to members in accessing community resources and social support services.
  • Supportive Role: Share personal experiences sensitively when appropriate, providing a relatable perspective for members.
  • Documentation: Complete all required documentation accurately and in a timely manner.
  • Training and Development: Participate in training programs to enhance skills and stay updated on best practices in care management.
  • Adherence to Policies: Follow all organizational policies and procedures to ensure compliance with regulatory standards.
  • Other Duties: Undertake additional tasks as directed by the Care Manager or supervisor to support the overall objectives of Tailored Care Management

North Carolina Certified Peer Support Specialists and Peer Support Paraprofessionals are individuals who:

  • Have a high school diploma or GED equivalency.
  • Are supervised by a Qualified Professional (QP) according to 10A NCAC 27G .0204.
  • Are not a family member of the individual who receives Peer Support services.
  • Paraprofessionals must meet requirements in 10 NCAC 27G .0104.
  • Complete 50 hours of Peer Support Specialist training.
  • Complete 20 hours of specialized training, in addition to the 50 hours required above.

Qualification/Education/Experience:

Peer Support must be delivered by individuals who have life experience of being diagnosed with a serious mental illness or substance addiction problem, but who are currently in recovery, are stable and living without relapses.

  • Minimum of a high school diploma or equivalent.
  • Individuals must be at least 18 years old
  • Must be an NC Certified Peer Support Specialist.
  • Previous experience in a healthcare or community support role preferred.
  • Excellent interpersonal and communication skills.
  • Ability to work collaboratively within a team and independently when necessary.
  • Empathy and understanding of individuals with behavioral health conditions.
  • They require supervision from a Care Manager.
  • Parent or guardian with two years of caregiving and Medicaid system navigation experience (excluding their own family member) or having two years of paid experience related to Tailored Care Management.
  • Other eligible candidates comprise Community Health Workers, former Community Navigators, individuals defined as Family Navigators by Trillium Health Resources, and those with personal experience of behavioral health conditions.
