Your Role as a Case Manager
In this role, you’ll provide targeted mental health case management services to help clients connect with medical, social, educational, vocational, and financial resources. You’ll advocate for clients, help resolve crises, and make a lasting impact in their lives.
What You’ll Do
- Support clients in navigating mental health services.
- Maintain accurate and timely documentation.
- Coordinate referrals & discharge planning with community partners.
- Meet with clients at least once every 30 days or as needed.
- Facilitate quarterly team meetings and advocate for clients.
- Collaborate with supervisors and participate in team meetings.
- Complete continuing education to stay up-to-date in the field.

Requirements
- Education: Bachelor’s degree in social work, psychology, nursing, or a related field.
- Ability to pass a criminal background check.
- Reliable transportation & valid driver’s license (for community-based roles).
Preferred Qualifications
- Experience working with diverse populations.
- Bilingual skills (highly valued!).
