Department: Harrisburg Family Medicine
Minimum Requirements:
Active IL RN license is required. BLS CPR and 1 year of population health nursing preferred.
Job Duties:
Coordination of population health efforts with clinical staff, ancillary departments, and clinic leadership. Review of clinical documentation to ensure health maintenance is current. Lead efforts to update health maintenance with any existing clinical care gaps. Provide population health education to providers and nurses as required. Work care gap reports to identify and schedule patients with appropriate providers. Track and schedule annual wellness visits and annual visits for all managed care plans. Other duties as assigned.
