Description : POSITION SUMMARY
The Clinical Coordinator will work under the direction of the Practice Manager and clinical executive leadership to ensure that all clinical aspects of the practice are being performed in a manner consistent with Fetter Health Care Network (FHCN) policies and procedures. The Clinical Coordinator will work on the floor as part of the clinical support team and will also be the primary point of contact for clinical-related initiatives, trainings, and other programs.
ESSENTIAL DUTIES AND RESPONSIBILITIES (Included but not limited to the following);
- Works with Practice Managers to ensure all clinical responsibilities are carried out in a manner consistent with FHCN policies and procedures.
- Serve as site VFC coordinator.
- Ensuring the assigned site(s) always has adequate stock of clinical supplies.
- Makes sure the assigned site(s) is submitting required patient documents for BCN, Choose Well, and other clinical programs in a timely manner.
- Coordinate with providers and patients to ensure completion and submission of patient access applications to include but not limited to Medata, Welvista, Access Health, Roper Charity and Tricounty Radiology.
- Ensures regular team huddles are performed and documented in compliance with PCMH standards.
- Serve as the site representative on agency committees such as Quality and Risk as determined by the practice manager.
- Attend clinical and non-clinical trainings as needed
- Training new clinical staff and ensuring up to date competency evaluations are performed on all nursing, CMA, lab, and other clinical staff as assigned.
- Collaborative problem solving and communication about clinical issues with the providers and agency leadership.
- Recommends, develops and implements clinical improvements for the practice.
- Oversees efforts for recruitment and staffing in collaboration with the Human Resources Department.
- Enhances operational effectiveness, emphasizing cost containment without jeopardizing innovation, or quality of care.
- Maintains compliance with Medicare, HIPAA, OSHA, and Labor Laws.
- Ensures maintenance of the physical building, including routine maintenance of equipment.
- Ensures ongoing commitment to excellent customer service.
- Completes required continuous training and education, including department specific requirements.
- Ensure the integrity of eMR by providing quarterly chart audits.
- Performs other duties as assigned
