SunnyBrook Home Care is looking for a Physical Therapist to join our team. This position is 32-40 hours per week. Relocation assistance possible for the right candidate. We are also willing to hire part-time with the support of the PTA. We pride ourselves on team work and flexibility! This is an opportunity to direct the therapy team to ensure the best outcome for each patient. The Physical Therapist uses clinical expertise, physical assessment, teaching and skilled therapy interventions to deliver high quality patient care in the patient’s home. The PT develops or contributes to the development of the home health plan of care under the direction of the physician and facilitates appropriate interdisciplinary involvement.

- Ability to pass a pre-hire background check, pre-employment health assessment, and drug screen
- Valid driver’s license, reliable transportation, valid insurance, and clean driving record
- At least one year of full-time work experience in a hospital or long-term care setting and/or home care experience required
- Graduate of a physical therapy degree from a physical therapist education program accredited by the Commission on Accreditation in Physical Therapy Education
- Currently licensed as a Physical Therapist in the state of Iowa.
- Candidates must excel in an autonomous work environment, be highly motivated, detail orientated, organized and technologically proficient.

- Understanding of current Medicare billing regulations
- Makes visits to the homes of patients requiring home health physical therapy services and assumes responsibility for an ongoing interdisciplinary assessment of the patient.
- Performs initial and comprehensive assessments on all patients in accordance with agency policies and procedures.
- Evaluates and assesses the patient’s level of functioning by applying appropriate tests.
- Treats clients to relieve pain, develop and restore function and maintain maximum performance using appropriate physical therapy treatments, modalities and procedures as stated in the physical therapy plan of care.
- Initiates diagnostic, preventative and rehabilitative procedures as appropriate to the patient’s care delivery
- Ensures quality and safe delivery of services to patients according to the care plan.
- Uses equipment and supplies safely, effectively, and efficiently.
