The Bereavement Coordinator is responsible for providing emotional support, grief counseling, and resources to hospice patients, families, and caregivers before and after the loss of a loved one. This role involves creating and implementing bereavement care plans, coordinating support groups, and ensuring compliance with hospice care standards.

- Develop and oversee individualized bereavement care plans for families and caregivers.
- Provide grief counseling and emotional support through one-on-one sessions and group meetings.
- Organize and facilitate bereavement support groups and memorial services.
- Collaborate with interdisciplinary hospice teams to address patient and family needs.
- Maintain documentation and records in compliance with hospice care regulations.
- Offer crisis intervention and support during critical situations.
- Provide education on grief and loss to hospice staff and community members as needed.
- Master’s Degree in Social Work (MSW) from an accredited institution.
- Licensure or certification as required by state regulations.
- Experience in grief counseling, hospice care, or related fields preferred.
- Strong interpersonal, communication, and organizational skills.
- Ability to work empathetically with patients and families during emotional times.
