Full Job Description: Case Manager – Adolescent Mental Health Services
Location: Greater Sacramento, CA
Pay: $27-$29 per hour Full-Time- 4 (10) hour shifts
The role: Make a lasting impact on the lives of at-risk youth. We are seeking a dedicated Case Manager to provide in-depth, detailed case management for adolescents facing mental health challenges. Available Positions
We have openings in both our Shelter Care and Short-Term Residential Therapeutic Program (STRTP), both of which provide a structured and supportive environment for adolescents in crisis. Our programs focus on stability, healing, and long-term success for youth in need.
Daily Life in This Role
- Work four 10-hour shifts per week, either Sunday–Wednesday or Wednesday–Sunday
- Hours are flexible, but we prefer 10 AM – 8 PM to ensure evening coverage.
- Manage a small caseload of 2-6 clients, allowing for highly individualized attention.
- Provide comprehensive case management, including service coordination, crisis intervention, and advocacy.
- Work in a highly supportive environment with low staff-to-client ratios, ensuring a manageable and rewarding workload.
What You’ll Do
- Provide direct support, coordinate services, and connect youth with resources.
- Advocate for clients and collaborate with schools, mental health providers, and juvenile justice agencies.
- Assist in crisis interventions, care planning, and transitions to new placements.
- Maintain detailed, in-depth documentation in electronic health records.
- Support students through IEPs, behavioral plans, and school reintegration.
Qualifications
- Bachelor’s degree in Social Work, Psychology, Juvenile Justice, or a related field and 2 years of experience working with youth-
- OR Associate’s degree in a related field and 4 years of experience.
- Experience in case management or social services preferred.
- Valid driver’s license.
- Strong attention to detail with the ability to provide in-depth case management.
- Ability to remain calm in high-pressure situations and support crisis response.
- Proficiency in Microsoft Office, Google Docs, and electronic health records.
