Job Description:

Essential Duties:
- Support connection to immediate, low-barrier treatment for patients with opioid use disorder (OUD) on a mobile unit operating throughout Harvey and surrounding south suburbs.
- Conduct initial patient assessments and ensure that patients are connected to ongoing treatment at community-based providers
- Support connection to case management services, including helping patients identify housing, employment, food, and other resources available to them in their community
- Assist with monitoring patient engagement and supporting clients early in the stages of change to move towards low-barrier, effective treatment plans.
- Describe FGC programs, services and eligibility requirements in laymen’s terms to people struggling with substance use disorder and those at risk of overdosing, as well as family members or friends of people struggling with addiction, informing and assisting them to gain immediate access to treatment.
- Develop referral relationships with shelter partners, homeless outreach teams, local task forces, hospital social workers, faith-based and local community organizations, and other community-based organizations as appropriate, serving as their liaison to accessing treatment services offered by FGC.
Work Schedule: Monday - Friday (7:00a - 3:30p)
Work Location: 15400 S. Page Ave., Harvey
Pay Rate: Up to $21.50 per hour

Experience and Skills:
Education and/or Experience: High School Diploma or equivalent. Certification as a Recovery Support Specialist or Peer Recovery Specialist required, OR currently working towards this certification. Experience in behavioral health, community health outreach, substance use harm reduction, withdrawal management programs, medication-assisted treatment, and/or community relations required. Bilingual in Spanish and English preferred.
Certificates, Licenses, Registrations: Certification from State of Illinois as a Certified Recovery Support Specialist (CRSS) or Certified Peer Recovery Specialist (CPRS) OR documentation of current coursework/internship to fulfill the requirements of this certification.
