Job Description:

****$1,000 SIGN ON BONUS****
1st $500.00: after 30 days of employment
2nd $500.00: after 6 months of employment
Essential Duties:
- Transport program patients for admission and discharge, medical and other community appointments. This is the primary responsibility, approximately 75% of work time.
- Maintain cleanliness and appearance of agency vans.
- Support treatment plans by reporting client observations on appropriate form (SAMMS / Incident Report/BOP documentation, etc.) - and supporting decisions concerning therapy, discipline, and privileges.
- Assists case manager/counselor with client activities by implementing scheduled and/or requested activities.
- Works cooperatively with members of the nursing and medical staff to monitor clients’ vital signs, observe urine drops, monitor clients, and chart all pertinent data.
Schedule: 12:00 noon - 8:30 pm (7:00 am - 3:30 pm on Mondays)

Work Location: 333 W. Evergreen St., Manteno, IL
Pay Rate: $17.50 per hour

Experience and Skills:
Education and/or Experience: High School Diploma / GED required: Experience in/with substance use, detox programs, medication-assisted treatment, behavioral healthcare and/or community relations encouraged to apply .
Certificates, Licenses, Registrations: Driver’s license and insurance required
