About the Role :
The Patient and Family Support Clinician who who holds a Florida State License serves as a compassionate resource and liaison, dedicated to supporting patients and their families throughout their treatment journey. This role is essential in enhancing the patient and family experience, ensuring that care is patient-centered, supportive, and responsive to individual needs. The Patient and Family Advocate will work closely with patients, families, and the care team to facilitate effective communication, provide emotional support, and address any questions or concerns about treatment options, hospital policies, Baker Act processes, and AMA protocols. This role also involves clinical work with patients and families, managing issues while providing dedicated, ongoing support.
Minimum Qualifications :
Master's degree in Social Work (MSW), Mental Health Counseling (MHC), or Psychology
Active Florida state license in field of study, required
2 years experience providing mental health services to individuals, and families
Current CPR Certification
Proof of Crisis Prevention Intervention (CPI) Training
Strong communication and interpersonal skills
Ability to maintain confidentiality and adhere to ethical standards
Responsibilities :
Patient Groups and Family Support : Builds trusting relationships with patients and families across all gender groups, offering guidance, support, and comfort. Address emotional and practical needs by listening and responding to their concerns.
Baker Act and Mental Health Advocacy : Assists patients and families through the Baker Act process, along with following FHE’s procedures, ensuring they understand their rights and the steps involved. Serves as a liaison to help families navigate mental health evaluations, resources, and support services.
AMA Support : Guides patients considering leaving Against Medical Advice (AMA) through the process, explaining potential risks and FHE’s AMA procedures. Ensure patients and families are fully informed and offer support to address any concerns or barriers that may impact care.
Biosocial Assessments and Develops Individual Treatment Plans : Completes bio assessments and creates tailored treatments plans for mental health patients.
Clinical Therapy : Assists with check-ins, individual, family and group therapy sessions as needed.
Program Orientation : Facilitates orientation for new admissions along with their family supports and assists in the welcome process. Ensures patients and family supports understand available services, treatment processes, and policies.
Communication Facilitation : Serves as a bridge between patients, families, and healthcare providers to improve understanding of medical information, treatments, and options.
Complaint Resolution : Listens to and address patient or family complaints or grievances in a respectful, empathetic manner, working toward solutions that meet the needs of all parties
Patient Advocacy : Acts on behalf of the patient and family to uphold their rights, preferences, and dignity in accordance with ethical standards and healthcare policies.
Documentation and Reporting : Maintains detailed and confidential records of all interactions with patients and families, documenting concerns and actions taken. Regularly report outcomes to the care team or management.
Interdepartmental Collaboration : Works closely with clinical and non-clinical teams to advocate for patients' needs and enhance the quality of care provided : assists with emergent clinical issues as needed across the facility.
