Minimum Requirements:
- 2 years of current Case Management RN experience.
- Current Louisiana RN License (compact license accepted).
- Valid Driver's License.
We are seeking a dedicated and experienced Case Manager RN to join our dynamic healthcare team in Lafayette, Louisiana. Unit Information:
- 200-bed facility, with responsibilities in the Medicine Unit or ICU/PCU Unit.
- Manage a caseload of 19 adult patients, providing high-level case management and care coordination.
Responsibilities:
- Assess, plan, and coordinate care for patients in both the Medicine and ICU/PCU units.
- Work closely with patients, families, and interdisciplinary teams to ensure appropriate care delivery.
- Ensure timely discharge planning and post-care follow-up to facilitate smooth transitions.
- Document all care plans and patient interactions in the EMR system.
- Advocate for patients' needs, ensuring that all care is aligned with clinical best practices and patient goals.
- Collaborate with physicians, social workers, and other healthcare providers to manage patient care.
