Currently looking for a compassionate and skilled Director of Case Management to oversee Case Management unit. This key leadership role will ensure that we continue to provide the best care to our patients through strategic planning, coordination, and continuous improvement. You'll be guiding a team of dedicated professionals, making a direct impact on the quality of care and patient experience.
What We're Looking For:
Must-Haves:
- Associate's Degree in Nursing
- Current RN license to practice in NC (or multi-state compact license)
- BLS Certification
- Ability to complete CCM Certification within the first year
Nice-To-Haves:
- Bachelor’s or Master’s in Nursing or a related field
- ACMA Certification
What You’ll Do:
- Lead the Case Management team in the areas of clinical resource management, discharge planning, patient advocacy, and medical necessity determination.
- Collaborate with medical staff, administration, and healthcare workers to streamline patient transitions and ensure the highest quality of care.
- Participate in strategic planning and performance improvement initiatives, continuously improving processes and patient outcomes.
