Responsibilities
- Lead and manage the case management team, ensuring high-quality service delivery.
- Develop and implement case management standards and protocols to enhance patient care.
- Conduct patient assessments to determine needs and develop appropriate care plans.
- Collaborate with healthcare providers and stakeholders to coordinate services effectively.
- Ensure compliance with HIPAA regulations and other relevant standards.
- Supervise staff performance, providing guidance and support for professional development.
- Monitor case outcomes and adjust strategies as necessary to improve service delivery.
Requirements
- Bachelor's degree in Nursing or related field; Master's degree preferred.
- Current nursing license required; experience in case management is highly desirable.
- Strong understanding of patient assessment processes and care planning.
- Excellent leadership skills with a focus on team development.
- Knowledge of Medicare regulations and healthcare standards is a plus.
- Experience in pediatrics or mental health counseling is beneficial but not mandatory.
