About the Role:
As a vital member of our healthcare team, you will:
- Patient Assessments: Perform a nursing assessment on all patients within 8 hours of admission; the data collected forms the basis of an individualized plan of care.
- Teaching Needs: Identify teaching needs and goals related to the diagnosis of each patient by collaborating with the patient/family and the interdisciplinary team.
- Communication: Provide patient information and status in a timely manner for the Initial Team Conference and for Re-evaluation Team Conferences on a weekly basis.
- Documentation: Responsible and accountable for accurate, succinct, and timely documentation of all assessments, interventions, and patient responses in the medical record in accordance with policy.
- Rehabilitation Participation: Participate in the patient rehabilitation program on an assigned unit.
- Direct Patient Care: Utilize the nursing process of assessing, planning, implementing, and evaluating patient care, prioritizing education for the patient/family/significant other based on identified healthcare needs.
- Positive Approach: Use a positive and encouraging approach to achieve patient goals and quality outcomes.
- Discharge Planning: Initiate discharge planning on all patients from the day of admission and begin patient teaching in anticipation of the discharge date.
- Continuity of Care: Communicate all pertinent information regarding changes in patient status between care providers to support continuity of care and patient safety.
- Consultation: Seek consultation from senior nursing staff for complex clinical decision-making.
- Training and Supervision: Provide direction, training, and supervision of ancillary nursing personnel while utilizing problem-solving techniques in decision-making, organizing, setting priorities, and managing time effectively.
- Delegation: Delegate appropriately and ensure personal and environmental safety according to hospital and nursing policies.
- Team Contribution: Positively contribute to unit functioning by providing calm direction and reassurance to staff, patients, and families.
- Communication with Management: Maintain communication with management and senior nursing staff regarding service needs such as necessary supplies, equipment (including repairs), and clinical training.
- Cost-Effective Care: Provide care in a cost-effective manner while promoting quality patient outcomes, monitoring supplies, and charges.
- Charge Responsibility: Assume charge responsibility following the orientation period as requested.
- Professional Growth: Identify and initiate personal professional growth, actively pursuing professional development through continuing education and self-study.
- Time Management: Prioritize activities and plan for effective use of time and staff in meeting patient needs.
- Additional Duties: Perform other duties as requested.
Qualifications:
- Valid RN License: Must hold an active and valid RN license in WV (or compact state).
- Clinical Experience: Proven experience in administering injections and IV therapy.
- Technical Skills: Familiarity with ventilator management and spinal tap procedures is a plus.
- Trauma Center Experience: Prior experience in a Level I Trauma Center preferred.
- Geriatric Care Knowledge: Strong understanding of geriatric care practices to provide effective support to elderly patients.
- Experience: Minimum 2 years in LTACH and clinic settings, with acute hospital experience.
- Licenses & Certifications: Current WV RN License (compact allowed), AHA BLS, AHA ACLS (must obtain by start date), COVID Vaccination Card (exemptions allowed).
- Soft Skills: A positive, encouraging demeanor and a knack for teamwork.
