Job Title: Social Worker (13-Week Contract)
Location: Alexandria, LA
Contract Term: 13 Weeks
Shift: Day Shifts, 5x8
*Rotating Weekends: Days (8:00 AM - 04:30 PM)
*No Call Required
Job Overview:
We are currently seeking an experienced Social Worker for a 13-week contract position in Alexandria, LA.
The Social Worker will be responsible for:
- Interviewing patients and families
- Conducting MDRs (Medical Discharge Reviews)
- Performing InterQual reviews on each patient.
This role requires expertise in discharge planning, observation reviews, and case management.
The Social Worker will float across the hospital, including working in various departments such as Women’s & Children’s (W&CH) and the Emergency Department (ED).
Minimum Requirements:
- 1 year of Social Work and Case Management experience.
- Current LA Social Worker License (LMSW)
- Experience with Midas/InterQual software.
- Valid driver’s license.
- Three supervisory references from the past two years of employment.
Key Responsibilities:
- Conduct face-to-face interviews with patients and families to gather information for care planning and discharge planning.
- Perform daily MDRs (Medical Discharge Reviews) for each unit.
- Complete InterQual reviews for all patients and contribute to discharge planning efforts.
- Participate in observation reviews and Discharge Planning Evaluations (DPEs).
- Document patient information and care plans using MediTech and Midas EMR systems.
- Float across the hospital, including working in Women’s & Children’s and Emergency Department as needed.
- Work collaboratively with interdisciplinary teams to ensure that patients are discharged in a timely and appropriate manner.
- Manage patient caseloads, ensure compliance with healthcare regulations, and advocate for the best interests of patients.
