You will work closely with members, caregivers, and healthcare providers to identify barriers to care and develop strategies to overcome them. This includes evaluating member needs, assessing social determinants of health, and recommending personalized care plans.
The ideal candidate will have a strong nursing background, excellent communication skills, and the ability to work independently in a fast-paced environment.
In this role, you will be responsible for:
