Key Responsibilities
This role involves working directly with members, caregivers, and healthcare providers to assess member needs and develop comprehensive care plans. You will evaluate barriers to care, recommend solutions, and collaborate with stakeholders to ensure timely access to care and services.
The ideal candidate will have a strong nursing background, excellent communication skills, and the ability to work independently in a fast-paced environment. They will also possess knowledge of social determinants of health, healthcare systems, and community resources.
In this role, you will:
- Evaluate member needs and develop care plans that address their unique challenges
- Collaborate with healthcare providers and community resources to ensure timely access to care and services
- Provide education and support to members and their families on disease processes, healthcare options, and benefits
- Maintain accurate records and communicate effectively with stakeholders
We offer a competitive salary range of $55,100.00 - $99,000.00 per year, as well as a comprehensive benefits package including health insurance, 401K, tuition reimbursement, and paid time off.
