Job Summary:
We are seeking a highly detail-oriented and organized Home Health Care Quality Assurance (QA) Specialist to join our team. This role is responsible for reviewing, auditing, and ensuring the accuracy and compliance of patient documentation, clinical notes, and records. The QA Specialist will work closely with clinicians, administrators, and compliance officers to uphold regulatory standards, improve documentation quality, and ensure adherence to Medicare, Medicaid, and other accreditation guidelines.
Key Responsibilities: Documentation Review & Compliance
- Audit and review patient documentation, visit notes, care plans, and physician orders for accuracy and compliance with home health regulations.
- Ensure all documentation meets Medicare, Medicaid, state, and agency-specific guidelines.
- Identify missing, incomplete, or incorrect information and work with staff to resolve documentation discrepancies.
- Maintain up-to-date knowledge of home health care regulations and industry best practices.
Quality Assurance & Process Improvement
- Conduct regular internal audits to identify trends and areas for improvement.
- Provide feedback and training to clinicians and administrative staff regarding documentation best practices.
- Assist in the development and implementation of quality improvement initiatives to enhance documentation efficiency and compliance.
- Monitor adherence to policies and procedures related to patient care and documentation.
Collaboration & Reporting
- Work closely with clinical teams, compliance officers, and agency leadership to ensure quality standards are met.
- Prepare detailed reports on audit findings and recommend corrective actions.
- Assist with regulatory surveys and accreditation processes by ensuring documentation readiness.
Training & Support
- Provide training and guidance to home health care staff on proper documentation practices.
- Develop and update educational materials related to compliance and quality assurance.
- Support new hires in understanding documentation protocols and regulatory requirements.
Qualifications & Requirements:
- Education: Associate’s or Bachelor’s degree in Nursing, Healthcare Administration, or a related field preferred.
- Experience:
- Minimum 2-3 years of experience in home health care, hospice, or a related healthcare setting.
- Experience in quality assurance, auditing, or compliance in home health care is highly preferred.
- Certifications: Certification in Home Health Compliance (CHHC) or related credentials is a plus.
- Technical Skills:
- Proficiency in Electronic Medical Records (EMR) systems SYNERGY
- Strong knowledge of Medicare, Medicaid, and ACHC/JCAHO home health care regulations.
